FORMS: Nearly all forms are available for electronic submission via this website; see specific form details, instructions, and links listed below.

1) Analysis Plan Form: All investigators planning projects or publications utilizing CANDLE core or ancillary study data must submit an Analysis Plan form and accompanying document. See Section F of the Collaboration Guidelines for more information.

2) Specimen Request Form: Any investigator wishing to use specimens from the CANDLE biorepository must submit a formal request specifying the samples they wish to obtain. This form must be submitted with the Analysis Plan Form. There is no electronic submission available for this document- please complete the PDF and fax a final copy to (901) 448-3770. 3) Abstract-Presentation Approval Form: Abstracts must have approved Analysis Plan before the abstract is submitted. Prior to submitting an abstract to a meeting or presenting an abstract at a meeting, the first author is responsible for completing an Abstract/Presentation Approval form. This confirms that all authors have had the opportunity to review the abstract, poster, or presentation prior to submission/ display. See section I of the Collaboration Guidelines for more information.

4) Manuscript Approval Form: A Manuscript Approval Form must be submitted by the first author prior to any manuscript submission, confirming that all authors have had the opportunity to review the manuscript and provide feedback. See section J of the Collaboration Guidelines for more information.

5) Additional Data Request Form: If, after an Analysis Plan has been approved, the investigator wishes to update their existing data set, an Additional Data Request form must be completed. See section L of the Collaboration Guidelines for more information.

6) Analysis Plan Amendment Form: If the objectives of the previously approved Analysis Plan evolve and deviate from the original plan, the first author is responsible for submitting an Analysis Plan Amendment. See section M of the Collaboration Guidelines for more information.

7) Student Request for Data Analysis Form: Students who wish to complete their own data analysis under the supervision of a CANDLE biostatistician may apply to do so through the submission of the Student Request for Data Analysis Form. See section N of the Collaboration Guidelines for more information.

8) Exit Consultation: Upon expiration or completion of an approved Analysis Plan, any investigator that has had access to CANDLE data must complete the CANDLE Exit Consultation Form. This form cannot be submitted electronically; a hard copy of the signed document must be submitted to the Emerging Science Coordinator. See section S of the Collaboration Guidelines for more information.
