Conflict Tactic Scales 2, (M2, CV2, CV4): Used to evaluate violence within families and intimate relationships.

Traumatic Life Events Questionnaire 2 (TLEQ), (M2, CV3): A brief self-report inventory that asks about current and prior exposure to 21 kinds of potentially traumatic events.

Achenbach Child Behavior Checklist (CBCL-3), (CV3, CV4): Evaluates maladaptive behavioral and emotional problems in children.

Social Support Questionnaire 6, (M2, CV2): Measures mother’s perceptions of social support.

Rosenberg Self-Esteem Questionnaire, (M2, CV2): Measures self esteem: a positive or negative orientation toward oneself; an overall evaluation of one’s worth or value.

Brief Infant-Toddler Social and Emotional Assessment (BITSEA), (CV1, CV2): Brief comprehensive screening instrument to evaluate social and emotional behavior in babies and toddlers.

Brief Symptom Inventory (BSI), (M2, CV1, CV2, CV3, CV4): Measures global symptomalogy in adults.

Edinburgh Post-natal Depression Scale (EPDS), (HV1, CV1): Used to screen for depression in  postpartum women in outpatient, home visiting settings, or at 6-8 week office visit.

Knowledge of Infant Development (KIDI), (M2, CV2): Measures comprehensive information on parents’ factual knowledge of parental practices, child developmental processes, and infant norms of behavior.

Household Questionnaire, (HV1, HV2, CV3, CV4): The household questionnaire counts the individuals (age, sex and the relationship to the participant or primary caregiver) living with the CANDLE participant. This questionnaire was created by CANDLE investigators for the specific purpose of CANDLE data collection.

Home Observation Measurement of the Environment (HOME) Inventory, (HV2): Measures the stimulation potential of a child’s early developmental environment.

Parenting Stress Index (PSI), (CV1, CV2, CV3, CV4): Quickly screens for stress in the parent-child relationship. It identifies dysfunctional parenting and predicts the potential for parental behavior problems and child adjustment difficulties within the family system.

Temperament Evaluation of Memphis, Pisa, Paris and San Diego (TEMPS), (M1, CV2): A screener for adult temperament.

Child Care Arrangements Questionnaire, (CV1, CV2, CV3): The Childcare Arrangements Questionnaire asks about the types of childcare the CANDLE child uses including how often these arrangements are used per week and the child to adult ratio in the daycare or home.  This questionnaire was created by CANDLE investigators for the specific purpose of CANDLE data collection.

State-Trait Anxiety Inventory, (CV3): Measures anxiety in adults, differentiating between the temporary “state anxiety” and the more general and long-standing “trait anxiety”.

NEO Personality Inventory, (CV3): Assesses personality, based on the Five-Factor model, the NEO PI-R measures the neuroticism, extraversion, openess to experience, agreeableness and conscientiousness factors.

CAGE Substance Abuse Screening Tool, (CV3): Brief Screener for Alcohol Abuse.

Center for Epidemiological Studies Depression Scale (CES-D), (CV4): Measures depressive feelings and behaviors during the past week.

Parental Incarceration, (CV4): This survey contains questions about whether mom or dad has been incarcerated during the child’s lifetime.  This questionnaire was created by CANDLE investigators for the specific purpose of CANDLE data collection.

Parenting Relationship Questionnaire, (CV4): Captures a parent’s perspective on the parent-child relationship.
